Chelsea broke the deadlock with 11 minutes remaining when Luiz met a free-kick from substitute Didier Drogba to divert the ball past Joe Hart.
Then, in stoppage-time, Ramires made sure the points as he danced through City's defense before finishing with aplomb past Hart.
